Please use this form when returning items for trade, credit, or advance replacement. You will need to contact our office for an RMA number before sending in your items. Please put this form in the package along with the returned items and write the RMA# on the outside of the box. This will ensure proper handling of the return. Please refer to our return and replacement policies for further information.
This form should accompany items sent in to North Atlantic, Inc. for repair whether the items are under warranty or not. To ensure a timely turnaround, please send this form with the parts to be repaired and write the number on the outside of the box.

North Atlantic, Inc. offers our customers the following payment terms: Company Check, Net Upon Receipt, or Net 30 Days. A personal guarantee must accompany the application. Please allow 7 to 21 days for approval terms. Original applications must be received by North Atlantic, Inc. for final approval.

For over $1,000 - Please complete and sign this form and return it along with a copy of the cardholder's driver's license and the front and back of the credit card. This will be kept on file as the original signature card and can be used for future orders. For orders under $1,000 - Only the card number, name as it appears on the card, and the card's billing address are needed.
